It's not 100% guaranteed to start, since the way a crisis spawns is based on a weighted chance determined every 5 years. Both of these can be really challenging. As a general rule of thumb, for x10 crisis you should have at least 1 million fleet power to be able to combat them, and 5 million is usually enough to win comfortably, and 10 million is strong enough to just curbstomp it without any real risk. It was not bugged in all versions. I fought the Contingency and the Unbidden a lot between 1.8 and 2.0.1 and they expanded at least a bit in all versions.
